This card is configured by ISA-PnP. It uses the Dream SAM9233 synth and the SAM 8905-1 effect processor. The waveblaster is mapped as a second MPU-401 interface. The on board ROM uses a version of the Roland Sound Canvas sound font.
The original board has a hardware bug on fast systems (late pentiums and faster) where the mainboard bios can freeze on warmboot when detecting ISA-PnP devices. The workaround is to replace capacitor C58 with a 16V 1 microF type. This is the orange cap on the photo.
The wavetable playback quality is quite good.
